All campers must have the following forms uploaded to Camp Site before camp begins:

1. Updated Immunization records

2. Universal Child Health Record 

3. Camp Waiver Form (required for enrollment and acceptance into Camp)

4. Camper Health Profile (required for enrollment and acceptance into Camp)

 

The following forms are only required for certain campers:

   1. Allergy Action Plan -required if your child has allergies

   2. Medication Authorization Form- required if your child will be taking medications during camp hours.

All forms can be found on your Parent Dashboard and must be completed and signed by a pediatrician (when required). No camper is allowed to camp without these forms completed. If you have any questions regarding forms, please email billing@acheinudaycamp.com.
