FORMS
All campers must have the following forms uploaded to Camp Site before camp begins:

Updated Immunization records

Universal Child Health Record
-
Camp Waiver Form (required for enrollment and acceptance into Camp)

Camper Health Profile (required for enrollment and acceptance into Camp)
The following forms are only required for certain campers:
1. Allergy Action Plan -required if your child has allergies
2. Medication Authorization Form- required if your child will be taking medications during camp hours.
All forms can be found on your Parent Dashboard and must be completed and signed by a pediatrician (when required). No camper is allowed to camp without these forms completed. If you have any questions regarding forms, please email billing@acheinudaycamp.com.
